These terms get used interchangeably at the counter — but they are distinct products with different formats, filters, and use cases.
Cigarillo

The tip may be flavored or coated.
Little cigar
Cigarette-sized, often filtered, wrapped in tobacco paper. Little cigars are nearly identical in size to king-size cigarettes (roughly 84 mm). They typically come with a filter and are wrapped in tobacco-based paper rather than leaf. Swisher Sweets Little Cigars (20-pack, $3.50–$5.00) are the most common example — the filtered version of the classic cigarillo. They burn for about 5–7 minutes and are designed to be smoked like a cigarette, filter and all.
In practical terms: if it looks like a cigarette and has a filter, it is a little cigar. If it looks wider, has no filter, and comes with a wood or plastic tip (or none at all), it is a cigarillo. The line can blur — Black and Milds, for example, are technically pipe tobacco cigars that share characteristics with both categories. But the key differentiator is size + filter: cigarillos are unfiltered and wider; little cigars are filtered and cigarette-sized.
